Updated to v1.05 - Tutorial Mode

This update adds an optional tutorial mode.  The tutorial adds a series of messages to the first floor of the dungeon to explain basic commands and gameplay and provide a little bit of guidance on making it through the first floor.  The tutorials are aimed at players who may not be overly familiar with this style of roleplaying game, as well as players who just want to jump in and learn the rules as they go.  Tutorials are 100% optional and can be turned on or off when starting a new game.  Existing saved games from 1.04 and earlier will have tutorials disabled.

The content and difficulty of the game are exactly the same with or without the tutorials; they are purely informational.  If you've tried Magic and Metal before and found it too unapproachable, this feature will hopefully provide a smoother introduction.

Other small changes and fixes:

- Moved the two fixed cloakrat encounters on 1F away from the doors, so that they do not attack by surprise.  This is intended to make the introduction to the game slightly more gentle.  The encounters have not changed.

- Escape now opens the field menu.  Also updated documentation to show that escape performs the cancel function, which was always the case but was not documented.

- L2 on gamepad now opens the help screen.

- Disable opening menus when an event is queued (i.e. the player is stepping into an event tile).  This is to prevent a lock that could happen if the key is pressed exactly as the event triggers.

- Fixed additional instances of player being able to cancel enemy combat text.

- Minor text additions on 1F.
